CA urban areas are often chopped up too much, IMO, so i'll be including UAs with populations going down to 50,000 people, if they're part of a cluster of 200,000+ people.

the Bay Area:

Contiguous UAs (those with development blended together to some extent at the borders) are grouped together:

------
San Francisco–Oakland, CA 3,515,933
San Jose, CA 1,837,446
Gilroy–Morgan Hill, CA 114,833
------

------
Concord–Walnut Creek, CA 538,583
Antioch, CA 326,205
Livermore–Pleasanton–Dublin, CA 240,381 (these ones are separated from the SF UA by parkland, with the closest inhabited areas in each being about a mile apart)
-------

-------
Vallejo, CA 175,132
Napa, CA 84,619 (these ones are separated from the SF and Concord UAs by the Carquinez strait, and is well connected to both via bridges, with the closest inhabited areas in each being less than a mile apart)
-------

-------
Fairfield, CA 150,122
Vacaville, CA 101,027 (these ones are separated from the Vallejo UA by parkland, with the closest inhabited areas in each a couple miles apart)
-------

-------
Santa Rosa, CA 297,329
Petaluma, CA 65,227 (these ones are separated from the SF UA by parkland, with the closest inhabited areas being a few miles apart)
-------

urban areas of 200k+ people within 200 miles of the Bay Area:

Sacramento, CA 1,946,618 (around 20 miles away from the Bay Area)
-Woodland, CA 61,133 (separated from Sac by a few miles of farmland)
-Davis, CA 77,034 (separated by a few miles of farmland)

Fresno, CA 717,589 (around 100 miles from the Bay Area)
-Madera, CA 81,635 (separated from Fresno by a few miles of farmland)

Bakersfield, CA 570,235 (around 200 miles from the Bay Area)

Reno, NV–CA 446,529 (around 150 miles from the Bay Area)
-Carson City, NV 61,629 (contiguous)

Stockton, CA 414,847 (around 10 miles from the Bay Area)
-Tracy–Mountain House, CA 120,912 (separated by a few miles of farmland)
-Manteca, CA 86,674 (contiguous)
-Lodi, CA 73,090 (contiguous)

Modesto, CA 357,301 (around 30 miles from the Bay Area)
-Turlock, CA 79,203 (contiguous)

There's also Merced (around 60 miles from the Bay Area), which doesn't quite make the cut off, at 150,052 people, but it's big enough to have a UC campus and future HSR stop, so I feel like I should include it.

There are also some regions within 200 miles where there are clustered, mostly non-contiguous urban areas, with a total population of 200,000+, but no single urban area with that many people:

the Monterey Bay Area (around 10 miles away):
-Salinas, CA 177,532
-Santa Cruz, CA 169,038
-Seaside–Monterey–Pacific Grove, CA 123,495
-Watsonville, CA 68,668

the Visalia area (about 150 miles away):
-Visalia, CA 160,578
-Tulare, CA 70,628
-Porterville, CA 69,862
-Hanford, CA 66,638

the Central Coast (around 150 miles away):
-Santa Maria, CA 143,609
-El Paso de Robles–Atascadero, CA 67,804
-San Luis Obispo, CA 56,904
-Arroyo Grande–Grover Beach–Pismo Beach, CA 50,885

Miami is pretty isolated, everything is over 100 miles away (because it takes 100 miles to get out of the urban area driving north):
Port St.Lucie: 115 miles 437,745 population
Bonita Springs/Estero: 133 miles 425,675 population
Vero Beach: 140 miles 174,292 population
Cape Coral: 158 miles 599,242 population
Palm Bay/Melbourne: 174 miles 510,675 population

almost 200k: Port Charlotte: 177 miles 199,998 population (just missed out by 2 people)

According to this source: https://www.freemaptools.com/find-population.htm

For Syracuse NY, places such as Ottawa, Buffalo, Rochester, Albany, Scranton, Springfield MA, Harrisburg, Allentown, Burlington VT, NYC, Toronto, Hamilton, Bridgeport and Hartford are within that radius in direct line terms. Some of these places are more than 200 miles in terms of a drive though. New Haven, Montreal and Erie PA are just outside of that radius.

Portland has 6 I could think of.

Seattle, Yakima, Tri Cities, Bend, Eugene and Medford/Rogue Valley. Aside from Seattle, all the smaller metros are right around 300k each. If you count taking a boat were within 200 miles of Victoria BC too.

Phoenix is pretty isolated as well:

Phoenix-Mesa-Scottsdale is the primary Urban Area for Phoenix metro, population 3,976,313 as of 2020 census. Urban Areas that meet the criteria:

1. Phoenix West-Goodyear-Avondale | 419,946 | 19 miles. Technically a separate Urban Area, though it's all contiguous and part of Phoenix Metro.

2. Tucson| 875,441 | 112 miles.

Edit: 3. Nogales | 232,303 | 181 miles.

If we were doing MSAs or CSAs, Yuma (203,881, 184 mi.), Prescott-Prescott Valley (236,209, 99 mi.) and Lake Havasu City-Kingman (213,267, 193 mi.) would qualify. But the Urban Areas for these metros are under the 200k population threshold; Yuma is 135,717, Prescott is 92,427, and Lake Havasu is 59,017.

So far it looks like the northeast corridor, the Great Lakes states, and coastal California are the densest regions.

Portland only has four UAs over 200K within 200 driving miles that I could find:

Seattle
Olympia
Salem
Eugene

And I think Minneapolis is another zero.

Is it the biggest lower 48 UA to blank out on this score?

I think it might be.

gainesville: 55mi (265k UA)
macon + warner robbins: 83mi/102mi (140k + 141k)
columbus: 107mi (267k)
chattanooga: 117mi (400k)
greenville, sc: 145mi (387k)
birmingham: 147mi (775k)
augusta: 147mi (431k)
montgomery, al: 161mi (254k)
huntsville, al: 180mi (329k)
asheville, nc: 197mi (285k)

nashville, charlotte, savannah and columbia are all within 250mi, knoxville is close at 214mi. athens' urbanized area is only 143k, surprisingly. if you add in the enrollment of UGA (40k) it's close to the cutoff, lol

Austin - 2,227,083

Killeen/Temple - 460,303 - 67 miles
San Antonio - 2,550,960 - 79 miles
Waco - 273,920 - 102 miles
Bryan/College Station - 264,728 - 103 miles
Houston - 7,066,131 - 162 miles
Dallas/Fort Worth - 7,573,136 - 195 miles
Corpus Christi - 429,024 - 194 miles

Technically, The Woodlands is part of Houston's metro, so it shouldn't really count as a separate one.

Urban Area and MSA seems like interchangeable terms but okay. Also I have a hard time separating Olympia, Tacoma and Seattle. Its all one big blob.
