Toronto's tallest unbuilt project was the 1971 Eaton's / John Maryon Tower proposal at 140 stories and 503m to the roof, or 686m to the top of the spire. The plans were abandoned because building a tower of that height at the time was considered unfeasible though, so it was arguably never really a serious proposal.
The tallest "serious" proposal probably would have been the Sapphire Tower in the mid-00s, at 337m to the top of the spire (or only 284m to the top of the roof, which would make it shorter than the existing tallest). I recall some excitement around the tower back when I first joined SSC/SSP, but I think we definitely lucked out in not having it built!

Sears Tower was 442m to the roof, surprised they didn't just shorten it a tad. that thing has a sort of O'Neill cylinder 1970s futurism vibe, not sure how it would age.

for Boise, the tallest proposal was a condo: Boise Place, 35 floors, 460 feet. Eventually, the Zion Tower was built on the property.

For Philadelphia, it was the 1500 foot American Commerce Center:
...which, although lamentingly unbuilt, was replaced with the Comcast Technology Center, which, at 1,121 feet, is still the tallest building in the US outside of NYC and Chicago:
Out here in Ventura County, the tallest proposals that I can think of were:
Channel Islands Center in Oxnard (2005), a twin 31 story complex with a 3rd tower that would have been 48 stories.
Both were killed eventually by the Great Recession.
In Santa Barbara, there once was a proposal for a 16 story apartment tower, well off State Street. This was in 1969. There was massive community opposition and today the block is a beautiful garden park.
