In SF it kind of depends if Philz is open or closed, ya know. Because if you're in the middle of a coffee and pastry, nothing can interrupt.
But when it comes to smoking, given the small minority who still do it, peer pressure pretty much takes care of violators of the various bans. They are hugely outnumbered and non-smokers can be very aggressive:
